1.0 INTRODUCTION
1.1 Definition
of accident
* An unfortunate incident that happens unexpectedly
and unintentionally, typically resulting in damage or injury.
* An event that happens by chance or
that is without apparent or deliberate cause.
1.2 Types of accident
Near Miss is an unplanned
event that did not result in injury, illness, or
damage but had the potential to do so. Only a
fortunate break in the
chain of events prevented an
injury, fatality or damage; in other words, a
miss that was nonetheless very
near.
1.3 Location
The accident in our video
basically happen at counstruction site.
1.4 Description of accident
A forklift want to lift up the
frame building after that the rope broke up. The
forklift use old rope or not
strong enough to lift up the building frame.
2.0 FINDINGS

3.0 Conclusion
We must has a responsibility to ensure
that we and our friends are safe at work. We must follow the rules even it look
like small things but somethimes the small thing can make trouble. Stay safe,
stay health .
Hazard
Definition of hazard,
A
hazard is any object, situation, or behaviour that has the potential to cause
injury,ill health , or damage to property or environment.
Health and safety hazards exist in every
workplace. Some are easily identified and corrected, while others are
necessary risks of the job andnmust be
managed in other way( for instance, by using protective equipment).
Most occupational hazards are inactive
or have a low potential of actually occurring, however employers must be prepared to deal with them since a
hazard becoming active can generate an emergency situation.
Hazards can appear in many occupational
circumstances. Some hazard are acute and pose an immediate danger to the health
and physical integrity of the worker or guests on the premises. Others take a
longer time to materialize and may have a cumulative effect, as in the case for
certain chemicals,vapour, dusts, and radiation that may lead to chronic medical
conditions following repeat or prolonged exposure.
Hazards can be classified as:
Physical Hazards: These are
the most common hazards and they included extremes of temperature, ionizing or
non-ionizing radiation , excessive noise, electrical exposure , working from
heights and unguarded machinery.
Mechanical Hazards:
These are usually created by machinery, often with protruding and moving parts.
Chemical Hazards:
These appear when a worker is exposed to chemical in the workplace. Some are
safer than others, but for workers who are more sensitive to chemicals, even common
solutions can cause illness, skin irriration, or breathing problems.
Biological Hazards:
These include the viruses, bacteria, fungus, parasites, any living organism
that can infect or transmit diseases to human beings.
Ergonomic Hazards:
Including considerations of the total physiological demands of the job upon the
worker, even beyond productivity, health and safety.
Psychosocial Hazards:
These may arise from a variety os psychosocial factor that worker may find to be unsatisfactory ,frustrating, or
demoralizing.
From our video hazard that can be
identify are:

4.0 Recommendation ( on how to reduce /
prevent an accident)
1.1 The importance of PPE
Safety is a major issue for day
laborers. Each year, accidents happen frequently in the construction industry
and often times it is due to the absence of Personal Protrctive Equipment (PPE)
or failure to wear the provided PPE. PPE is equipment that will protect workers
against health or safety risks on the job. The purpose is to reduce employee
exposure to hazards whe engineering and administrative controls are not
feasible or effective to reduce these risks to acceptable levels.
Head Proctection
Hard hats are common on construction
sites. Just passing by a site, you
can usually identify workers by their hard hats. They are designed to
protect
against flying or falling objects that would otherwise impact or
penetrate the
worker. Some hard hats are equipped with accessories such as face
shields
and earmuffs. Hard hats should be well-fitted, those that are too larger
or
too small are inappropriate for use.
Eye and Face protection
Eye and face protection are
equally as important as head protection. Safety
googles, spectacles and full face shields can give you the protection
needed
for the eyes and face. Metal work, wood-work,hot-work and air-tool
operations all require this type of protection. General laborers can
also
benefit from safety googles since there is usually debris on
construction sites.
Respiratory protection
Respiratory
protections is vital on sites where toxic substances are present.
Sometimes what you can’t see can hurt you. Respiratory protections like
respirators are designed to protect you from dust, fumes, paint spray,
pesticides and other dangerous substances that could cause permanent
impairment. Respiratory protection should be used in environments with
air
contaminants. In work enviroments, respirators are relied upon when
adequate ventilation is unavailable or other engineering control systems
are
not feasible or inadequate.
1.1 Safety signage
Safety
signs are crucial in any work environment. The primary importance of displaying
safety signs is to prevent injury and ensure staff and visitors are well aware
of the possible dangers and hazards ahead in certain situations or enviroments.
Without signs, many employees would lack the necessary direction in times of
crisis, and employers might find themselves in significant legal difficulties
if any accidents were to arise as a result.
